  This is a case of a 14 year old girl who came with chief complaints of: Shortness of breath since 2 days Fever since 1 day Abdominal pain since 1 day Vomiting 1 episode HOPI:  The patient was apparently asymptomatic 4 years ago, then she had a fever with generalised body weakness and polyuria and was diagnosed having type 1 DM and started on insulin. 2 years ago patient had similar complaints after an episode of fever and was admitted at our hospital and was treated here.   2 days ago then she developed sudden onset shortness of breath since 2 days, gradually progressive and progressed to stage 4. Shortness of breath started after patient missed taking 2 doses of insulin. No orthopnea, no PND.  Fever since 1 day, high grade associated with chills and rigors, relieved on taking medication, no diurnal variation. E.Likhitha Roll no.34           A 55 year old male ,who is a toddy maker by occupation and residency of chotuppal ,came to the department with ,     C/O Shortness of breath since 2 days             Fever since 2 days             Cough since 1 day. HISTORY OF PRESENTING ILLNESS:       Patient was apparently asymptomatic 3 years back ,then he developed shortness of breath of Grade-2 (acc. to MMRC ) the patient cannot walk beyond 500m and as he was a toddy maker ,he develops SOB on uphill,which is insidious in onset and not progressed till 10-04-23 and it is not associated with orthopnea and paroxysmal nocturnal dyspnea ,aggravated on exposure to cool temperature,by eating sweet and sour food,on exposure to dust in traffic and it is relieved on medication . On 10-04-23,patient developed SOB of Grade-4(acc.to MMRC),he developing SOB even at rest ,which is sudden in onset and non progressive associated with wheeze ,due to this he was admitted in hospital .   CASE; E.Likhitha  Roll no.34    A 70 year old  female,resident of Erupu village of suryapet district,home maker by occupation came to the hospital with,   C/O,  Loin pain on Right side since 5 months, Pedal edema on both legs since 2 months.
